Checklists to help you implement the four Continuity of care policies
The OMA has developed six checklists to help you understand and implement the continuity of care policies.
The checklists organize expectations from all four policies into common themes so that you can easily apply them to your everyday tasks and practice contexts.
They have been updated to reflect the new information from the Advice to the Profession document.

Doing consults, preparing and distributing consultation reports, receiving and reviewing consultation reports, preparing for temporary absences from practice and practicing in walk-in clinics.
Handover and discharge checklist
Handing over responsibility for patient care, having discharge discussions with patients, completing discharge summaries and distributing discharge summaries.
